FORMS YOUR CHILD NEEDS

Forms required for ALL Children (click to download forms):

1. Application for Enrollment
          - Must be completed BEFORE your child attends the CDC.


2. Discipline Policy
          - Must be completed BEFORE your child attends the CDC.


3. Medical Report
          - Must be completed within 30 DAYS of your child's start date.


4. Immunization Records
          - Must be completed within 30 DAYS of your child's start date.


5. Letter Indicating Receipt of North Carolina Child Care Law
          - Must be completed on your child's start date.


6. Parental Agreement (Last Page of Parent Handbook)
          - Must be completed on your child's start date.


7. Food Program Application
          - Must be completed on your child's start date.

Forms Required for Infants (click to download forms):

1. Feeding Schedule (15 Months and Younger)
          - Must be completed BEFORE your child attends the CDC.


2. Safe Sleep Policy
          - Must be completed BEFORE your child attends the CDC. (the waiver may be signed only if your child is 6 months or older.)


3. Parent Letter Regarding Formula for Infants (12 Months and Younger)
          - Must be completed on your child's start date.

Forms Required for One and Up:

1. Permission to Play outside Fence (One Year and Older)
          - Must be completed BEFORE your child attends the CDC. If you have any questions regarding this form, please ask.


2. Field Trip Permission Slip for ALL Filed Trips (Older 3's and Up)
          - Must be completed BEFORE your child attends a field trip.
